Care Program Requirements

The Care Program is intended for clients who are actively engaged in their therapeutic progress and committed to improving their overall health and wellness. To maintain eligibility, participants must meet the following requirements:

• Attend scheduled appointments consistently.

• Must complete 3 sessions at full cost to assess goal plans, therapist/client compatibility and clients dedication to their well being.

• Actively participate in recommended home care practices.

• Maintain open communication with their therapist regarding progress, concerns, changes in health status, and treatment goals.

• Demonstrate a commitment to making reasonable lifestyle adjustments that support therapeutic outcomes.

• Remain in good standing with all payment and cancellation policies.

• Arrive on time and prepared for appointments.

Program Review & Continued Eligibility

Participation in the Care Program may be reviewed periodically to ensure the program is being used effectively and that therapeutic goals are being actively pursued. Clients who consistently fail to participate in recommended home care, do not maintain communication regarding their treatment progress, repeatedly miss appointments, or otherwise fail to meet program requirements may be removed from the program and returned to standard session pricing.

The purpose of these requirements is to encourage a collaborative therapeutic relationship in which both the client and therapist actively contribute to achieving the best possible outcomes.
